  Below is a list of required documents during the application

  Prepare required documents before application

  • A copy of business registration certificate issued by the Competent Authority (RDB,RCA,…) /Kopi y’icyemezo cy’iyandikwa ry’ubucuruzi gitangwa na RDB cyangwa RCA;
  • A copy of premises ownership permits or a copy of lease agreement/ Kopi y’icyangombwa cy’ubutaka inyubako iherereyemo cyangwa icyemezo cy’ubukode bw’inyubako;
  • List of personnel with their valid medical certificates (Obtained from public hospitals. i.e health centers and private’s clinics are excluded)/ Urutonde rw’abakozi akoresha na kopi z’ibyemezo bya muganga bigaragaza ko batarwaye indwara zandura;
  • N.B/Icyitonderwa: This certificate is obtained only from public hospital and is given with the laboratory test results/ Iki cyangombwa gifatirwa gusa ku bitaro bya Leta kandi kikagendana n’ibisubizo bya laboratwari
  • Proof of payment of license application fees /Inyemezabwishyu y’amafaranga y’uruhushya, click here for declaration .

  Below is a list of required documents during the application

  Prepare required documents before application

  1. Provisional authorization (for new garage construction)/ Uburenganzira bw’agateganyo (ku igaraji rishya rigiye kubakwa):
     • A copy of business registration certificate issued by the Competent Authority/Kopi y’icyangombwa cy’iyandikwa ry’ubucuruzi mu rwego rubishinzwe
     • Architectural drawings of the garage/Ibishushanyo by’ubuhanga byerekana imiterere y’igaraji;
     • Environmental impact assessment certificate from the Competent Authority/ Inyandiko igaragaza inyigo nsuzumangaruka zaho igaraji rigiye kubakwa;
     • Approval document from the district office that the site is complying with the district physical masterplan. This document can be a non-objection letter or a construction permit/Inyandiko igaragaza ko aho igaraji rigiye kubakwa hubahiriza imikoreshereze y’ubutaka nk’uko bigenwa n’akarere rigiye kubakwamo. Iyi nyandiko ishobora kuba ibaruwa cyangwa se uruhushya rwo kubaka.
     • Proof of payment of an application fee/Ikigaragaza ko bishyuwe amafaranga y’ubusabe;
  2. Registration of new constructed garage / Kwandikisha igaraji rishya
     • A copy of provisional authorization/Kopi y’uruhushya rw’agateganyo;
     • A copy of garage insurance/Kopi y’ubwishingizi bw’igaraji;
     • Proof of payment of application fee/Ikigaragaza ko bishyuwe amafaranga y’ubusabe
  3. Registration of an operating garage (First registration)/ Kwandikisha igaraji risanzwe rikora (kwandikwa bwa mbere):
     • A copy of business registration certificate issued by the Competent Authority/Kopi y’icyangombwa cy’iyandikwa ry’ubucuruzi mu rwego rubishinzwe
     • Architectural drawings of the garage/Ibishushanyo by’ubuhanga bigaragaza imiterere y’igaraji:
     • Environmental impact assessment certificate from the Competent Authority/ Inyandiko igaragaza inyigo nsuzumangaruka zaho igaraji rigiye kubakwa;

  4. Renewal for garage registration certificate/ Kongeresha agaciro k’urupapuro rw’iyandikwa ry’igaraji ryari risanzwe ryanditse:
     • Copy of expiring registration certificate/Kopi y’uruhushya ruri guta agaciro
     • Copy of garage insurance/ Kopi y’ubwishingizi bw’igaraji
     • Proof of payment of application fee/Ikigaragaza ko bishyuwe amafaranga y’ubusabe
  5. Provisional authorization for altering the layout/setting for a registered garage by RICA/ Uburenganzira bw’agateganyo bwo guhindura imiterere y’igaraji risanzwe ryaranditswe na RICA:
     • A copy of existing registration certificate issued by RICA/Kopi igaragaza icyangombwa gitangwa na RICA igaraji risanganwe;
     • Rehabilitation permit issued by Competent Authority/Uruhushya rwo kuvugurura rutangwa n’inzego zibifitiye ububasha
     • New architectural drawings of the garage/Ibishushanyo by’ubuhanga bishya byerekana imiterere y’igaraji.
     • Proof of payment of an application fee/ Ikigaragaza ko bishyuye amafaranga y’ubusabe
  6. Registration of an altered garage (after construction)/ Kwandikisha igaraji ryahinduriwe imiterere (nyuma yo kubakwa):
     • A copy of provisional license /Kopi y’uruhushya rw’agateganyo
     • A copy of garage insurance/Kopi y’ubwishingizi bw’igaraji
     • Proof of payment of an application fee/Ikigaragaza ko bishyuye amafaranga y’ubusabe

  1. Milk collection center required documents
     • A copy of business registration certificate issued by the Competent Authority / Kopi y’icyemezo cy’iyandikwa ry’ubucuruzi gitangwa na RDB cyangwa RCA/RRA
     • A copy of ID of the business operator (for individuals)/ Kopi y’indangamuntu (Iyo ari umuntu ku giti cye);
     • A copy of premises ownership permits or a copy of lease agreement/ Kopi y’icyangombwa cy’ubutaka inyubako iherereyemo niba ari iyusaba uruhushya cyangwa icyemezo cy’ubukode bw’inyubako
     • Approval from the district office that the MCC/MAP is allowed to operate in the given area/ Icyemezo cy’Akarere kigaragaza ko ikusanyirizo ryemerewe kubakwa no gukorera aho rigiye kubakwa.
     • List of personnel and their medical certificates/Urutonde rw’abakozi bakira amata n’ibyangombwa byabo bya muganga
     • N.B: This certificate is obtained only from public hospital and is given with the laboratory results/ Iki cyangombwa gifatirwa gusa ku bitaro bya Leta kandi kikagendana n’ibisubizo bya laboratwari
     • Proof of payment of an application fee/Ikigaragaza ko bishyuwe amafaranga y’ubusabe;

  • A copy of authorization for transport of goods issued by a Competent Authority /Kopi y’cyangombwa cyemerera imodoka gutwara ibintu gitamgwa n’Urwego rubifitiye ububasha
  • Calibration certificate (if it has cooling compartment)
  • A copy of vehicle inspection certificate (controle technique) issued by the Competent Authority /Kopi ya controle technique itangwa n’Urwego rubifitiye ububasha
  • A copy of vehicle registration certificate (carte jaune) /Kopi ya carte jaune y’imodoka
  • Copy of ID of business owner(s) or a copy of company registration/cooperative certificate RDB/RCA/RRA /Kopi y’irangamuntu ya nyir’ubucuruzi cyangwa Kopi y’icyangombwa cy’ubucuruzi gitangwa na RDB/RCA/RRA
  • Valid medical certificates for the driver obtained from a recognized public hospital /Icyemezo cy’umushoferi cyatanzwe na Muganga wa Leta kerekana ko atarwaye indwara zanduza
    • NB/Icyironderwa 1: The medical certificate is accompanied by the laboratory test results indicating undertaken exams and results. / Icyemezo cya muganga kigomba guherekezwa n'ibisubizo byavuye mu isuzuma ry’ibizamini.
    • Medical check-up shall include the following conditions or diseases: Kwisuzumisha kwa muganga bigomba kwibanda ku ndwara zikurikira:
      
      1. Tuberculose / Igituntu
      2. Hepatite / Indwara ya hepatite
      3. Gastro-intestinal bacteria (E. coli, Salmonella) / Indwara ziterwa n’udukoko zo mu mara.
